By Email:  Simply EMAIL us your your name, address, phone number, date of birth, and the name/location of your current pharmacy to: [email protected] We will take care of the rest! You will receive an email confirmation of this transfer.

By Phone:  Simply CALL us with your your name, address, phone number, date of birth, and the name/location of your current pharmacy at:613-842-7455. We will take care of the rest! You will receive a courtesy phone call confirmation of this transfer.

By Fax:  Simply FAX us with your your name, address, phone number, date of birth, and the name/location of your current pharmacy at:613-842-7453. We will take care of the rest! You will receive a courtesy phone call confirmation of this transfer.
